If you do not know the official name of the certificate, describe in your own words what needs to be confirmed and what for. The office will find the right form.

How to send it

What almost always counts is not the date on the letter but the date it arrives. Send it by registered post, or hand it in and ask for a stamp on your own copy — that gives you the proof the deadline is counted from.
